Pay Rate: £37 per hour (assignment rate)

Contract: Locum, 3-Months

Requirements of the Social Worker:

• Degree or equivalent in social work
• Current Social Work England registration
• Significant frontline experience in Adults Services

Main Duties:

• Complete care act assessments to determine eligibility
• Complete care and support plans, where eligible needs are identified.
• Ensure community resources are drawn upon and work is strength based and progressive in nature
• Complete initial reviews/ annual reviews
• Work with those under 18 to plans for their adult support at 18 (via assessments)
• Work with those with Autism and no learning disability. Work may also include working with those with other neurodiversity’s or vulnerabilities, not supported through other social care teams.
