Overview

We are looking for a dynamic and experienced Social Worker to join our Hospital Discharge Team, supporting adults with timely, safe discharges and person-centred care planning.

Key Responsibilities
  • Carry out assessments & reviews under the Care Act
  • Be present on-site at the hospital when required
  • Attend Decision Support Tool (DST) meetings
  • Support with hospital discharges and multi-agency working
  • Work closely with health professionals & families
Requirements
  • Qualified Social Worker (SWE registered)
  • Minimum 2 years of hospital social work experience
