What you will do:

1. Conduct comprehensive assessments of needs and risks, including safeguarding investigations under Section 42 of the Care Act 2014.
2. Collaborate with service users and carers to develop and implement personalised care and support plans.
3. Coordinate care by working closely with partners from health, housing, and the voluntary sector.
4. Respond to safeguarding concerns accurately and swiftly, taking appropriate protective actions when needed.
5. Support individuals in decision-making, applying the Mental Capacity Act when necessary to ensure their best interests.
6. Maintain clear, accurate case records and fulfil all statutory duties with diligence.
7. Participate in multi-agency meetings and case conferences to advocate for service users.
8. Promote a person-centred, strengths-based approach to practice, enabling increased independence and resilience.
