Social Worker - Adult Social Care West Team

Sanctuary Personnel has a new permanent position for a Social Worker within the Adult Social Care West Team, based in West Berkshire. This is a full-time role with salary up to £41,418 per annum.

Responsibilities
  • To provide people with good quality advice and information relating to care and support.
  • To receive and respond suitably to contacts from people who need support and their carers.
  • To work with people to connect them to community resources, with people in crisis to stabilise their situation, to meet long-term care and support needs and review care and support arrangements.
  • To ensure that service users are safeguarded by acting in line with the Care Act and Mental Capacity Act.
  • To assist service users and carers to construct a Support Plan which meets their needs, outcomes and covers identified risks, offering flexibility and choice through engagement with families, the community, activities, voluntary organisations, and provider services.
  • To develop relevant expertise and knowledge appropriate to the role of a Social Worker.
  • To provide social work expertise to other team members and undertake supervisory responsibilities as directed by the Team Leader.
  • To record all records/reports accurately and clearly in the required formats.
  • To promote equality and to treat everyone with fairness and dignity.
  • To comply with health and safety requirements and the WBC Health and Safety policy.
  • To ensure own practice meets the required standard by following relevant policies, procedures and guidance.
  • To actively participate in supervision, appraisal and relevant training.
Qualifications
  • A Social Work Qualification or equivalent.
  • Understanding of relevant legislation.
  • Relevant Social Work experience.
