What will your responsibilities be?

  1. Support adults and young people with learning disabilities by completing Care Act Assessments and reviews.
  2. Work collaboratively within a multi-disciplinary team, including Adult Social Care and NHS Health staff.
  3. Manage safeguarding cases, assess risk and coordinate care management effectively.
  4. Engage with service users, families and partner agencies to promote person-centred support.
  5. Demonstrate strong communication and organisational skills, and a commitment to improving outcomes for individuals with learning disabilities.

Qualifications & Experience:

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level and have at least 2 years of post-qualified experience.
