What will your responsibilities be?
In this role, you will support the delivery of Health and Social Care functions for adults and carers in Wirral. You will ensure compliance with the Care Act 2014, promoting strength-based practice, personalisation, and social inclusion. Your responsibilities will include assessing, accessing, and implementing social care services for adults with complex physical and mental health needs, older people, and carers. Additionally, you will provide supervision and practice support to ASYE/newly qualified social workers and Care Navigators. You will offer guidance, advice, and information to support adults and carers with diverse and complex needs, ensuring they receive the necessary care and support to improve their quality of life.
Qualifications & Experience:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level and have at least 2 years of post-qualified experience in social work.
