Social Worker – Home First Team – Swansea (UK) – £38.43 per hour (Hourly)

This is an exciting opportunity for one full-time Social Worker to join our dynamic Home First Team within Swansea Council on a temporary basis for an initial period of 3 months. This role will allow you to make a real difference in the lives of those in our community. • Work collaboratively within the Home First Team and local hospitals to facilitate safe and timely discharges for patients.
• Conduct comprehensive assessments of individuals’ needs in hospital and community settings.
• Develop and review care plans, ensuring they are tailored to meet the specific needs of each individual.
• Liaise with multidisciplinary teams, families, and external agencies to coordinate effective care and support.
• Support individuals in regaining independence and integrating back into their communities.
