Description

Swansea Council is seeking a committed and experienced Social Worker to join our Community Mental Health Team (CMHT). This is a fantastic opportunity to contribute to a multidisciplinary team dedicated to providing high-quality mental health support across the community. Based in Swansea, this role involves a hybrid of office-based work, home visits, and hospital assessments.

Responsibilities

  • Deliver statutory mental health services in line with the Social Services and Well-being (Wales) Act 2014 and the Mental Health Act 1983.
  • Undertake assessments and develop person-centred care plans, working in partnership with service users, families, and carers.
  • Manage complex cases and act as care coordinator, ensuring effective risk management and safeguarding interventions.
  • Prepare and present reports, including for Mental Health Tribunals.
  • Work collaboratively with NHS colleagues and other professionals to support recovery-focused care.
  • Maintain accurate, up-to-date records and contribute to service development.

Requirements

  • Qualified Social Worker registered with Social Care Wales.
  • Mental health social work experience essential, ideally within a CMHT or similar statutory setting.
  • Confident in completing reports and attending Mental Health Tribunals.
  • Good working knowledge of WCCIS would be advantageous.
  • Strong communication, organisational, and partnership-working skills.
  • Must have a valid UK driving licence and be able to travel to clients across Swansea.
  • Ideally located within or near Swansea, or willing to travel for the duration of the contract.
