Durham County Council is seeking a compassionate Social Worker for a temporary maternity cover role (30 hpw) in a hospital social work team. You will balance discharge planning with safeguarding responsibilities to support timely, person-centred care for patients and their families.

The post holder will have a key role in ensuringan appropriateandtimelyresponse to supporttimelydischarge from hospital.
The post holder willbe requiredto contribute to the 8am – 8pm, 7 days per week, service delivery model, to include working occasional weekends and on-call arrangements over Bank Holidays.
The team enjoy excellent working relationships with colleagues in health settings and have a close working relationship with the other Integrated teams and statutory and non-statutory organizations.
