Job summary

Southampton City Council's City Wellbeing Team is seeking experienced social workers to support adults with physical disabilities or dementia in the community, at home or in care settings. The role involves delivering statutory services and working with families and carers.

Job description

We are currently looking for experienced social workers to our City Welling Team and are inviting applications from individuals with 1 years post qualifying experience.

About the team:

You will joinan energetic and supportive team striving to support people in Southampton live active, safe and independent lives. The City Wellbeing Team supports adults living in the community with either a physical disability or a diagnosis of dementia and delivers statutory services to those individuals, families and carers who are already in receipt of support. This can be delivered either in their own home or in a residential or nursing placement.

The social work roles and responsivities include undertaking reviews under the Care Act 2014, Mental Capacity Act 2005 assessments, contributions to multi-agency planning, provide interventions to individuals and their families or carers. Negotiating and coordinating resources and conducting s.42 safeguarding enquires.

Salary: The salary band for this role is £43,150 - £47,664 per annum. The starting salary is £43,150 with annual progression through the salary bands.
