Working Hours: 3 x 35 Hours per week

Contract Type: Permanent / Full-Time

Additional Payments: 6.7 % Flexibility Payment, Essential Car User Allowance

This role is subject to -

• Qualification - Social Work Qualification and registration with Social Work England.
• Medical Clearance - Pre-placement health assessment (PPHA)
• Car Documentation - This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance. Must be a car driver
• DBS (Disclosure and Barring Service) Clearance - This post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here. The hospital discharge to assess team support citizens through the hospital discharge processes. Our input, if required, starts in hospital and continues into the community where a full assessment will take place in an appropriate setting.

You must be willing to be flexible and able to work at a fast pace. You will need to be a confident practitioner with a good understanding of the social work role to ensure effective multi-disciplinary working.

We work flexibly throughout the whole service, this includes; D2A, Crisis response, intermediate care and the hospital. Social workers will work within the multi disciplinary team both in and out of the hospitals to ensure a holistic approach to assessment. We operate a 7 days service.
