About the Role: Hospital Team
We are seeking two experienced and motivated Social Workers to join theLambeth Hospital Social Work Team, supporting Lambeth residents admitted toKing’s College Hospital (KCH),Guy’s and St Thomas’ (GSTT), and hospitals outside the borough. This is a fast-paced, collaborative environment where you’ll play a vital role in delivering strength-based, preventative support and safeguarding interventions.
Key Responsibilities:
Hybrid Working:Required on-site 3 days per week, with the option to work remotely for 2 days.

Join the Intermediate Care Lambeth (ICL) At Home Team—a collaborative, multidisciplinary service designed to provide urgent, community-based care that prevents unnecessary hospital admissions. We bring together staff from Lambeth Adult Social Care and Guy’s & St Thomas’ Hospital Trust to make a meaningful difference in the lives of Lambeth residents.
The Role:
We are looking for an experienced and compassionate Social Worker to join our dynamic team. As part of this integrated service, you will play a key role in supporting individuals in the community, ensuring their needs are met through person-centred care and collaborative teamwork.
Key Responsibilities:
Rehabilitation & Reablement Services: Identify and support cases that require the Rehab & Reablement pathway, helping residents regain their independence.
Key Requirements:
