We are looking for Social Workers with empathy and the drive to make a difference, finding creative ways to support people to achieve their goals and live as independently as possible in a place they call home.

We need a permanent, part-time Social Worker to work across Lancashire as part of our Urgent Care Acute and Prisons Service.

Living Better Lives in Lancashire

This is an exciting time for us. We've launched Living Better Lives in Lancashire – our new strength-based vision for social care.

We're driving forward innovative practices that will make a difference in our communities, and we need new social workers to help us achieve our vision of supporting people to achieve their goals and live independently in a place they call home.

Our team provides a single point of access to community intermediate care services, including reablement and residential rehab, seven days a week. We also support hospital discharges and work with the wider multi-disciplinary team to prevent hospital admissions and help people remain at home. Additionally, we utilize the Discharge to Assess model to ensure assessments are conducted away from the hospital environment.

This key role involves holistic, face-to-face Care Act Assessments, reviews, and support planning, using a strengths-based approach to ensure people receive the right support at the right time, enabling them to achieve their optimum level of independence and live their life as they choose.
