Northumberland County Council is seeking a passionate Social Work Support Assistant to join the Care and Support Team, delivering high-quality services to adults with disabilities, long-term conditions and mental health needs across North Northumberland.

Based in Berwick-upon-Tweed and Alnwick, you will complete strengths-based assessments, support safeguarding, promote self-directed care and work with a multidisciplinary team including Social Workers, Care Managers, GP Practices and District

Formación

• Level 3 qualification in social care or equivalent.
• Experience of working with adults in a social care setting.
• Driving licence and access to a car for work purposes.
• Knowledge of legislation relevant to mental health and adult social care.

Responsabilidades

• Completing strengths-based needs assessments, ensuring safeguarding considerations are embedded throughout.
• Reviewing eligibility for Continuing Healthcare and supporting effective care planning.
• Promoting self-directed support, empowering people to make informed choices about their care and support.
• Working collaboratively within a multidisciplinary team, including Social Workers, Care Managers, GP Practices, Community Health Services and District Nurses.
• Supporting individuals to live as independently as possible within their local communities.
• Managing risk appropriately and helping to safeguard vulnerable adults from harm.
• Building positive relationships with service users and partners while maintaining accurate records and using IT systems effectively.
