Social Work Support Assistant, Adult Social Care – Permanent, Full time (37 hours), countywide Learning Disabilities Team.
We have an exciting opportunity for a Social Work Support Assistant / Care Manager 1 to join our Countywide Learning Disabilities team. We are looking for a passionate person, keen to deliver high-quality service.
As a key worker for adults with care needs and learning disabilities over 18 years of age, you will be the main point of contact for support and advice to them and their families. You will ensure their safety and help them live as independently as possible. Responsibilities include completing needs assessments, safeguarding, reviewing healthcare needs, and promoting self-directed support. You will work within the countywide Learning Disability team, collaborating with social work colleagues and health professionals, including nurses, psychologists, and primary health staff.
You should have a strong commitment to enabling service users to live independently and think creatively about their options. The role involves managing risks and protecting service users from harm. You need to work well independently and in a team, with good time management, interpersonal, advocacy, communication, and IT skills.
