A leading community health provider in Canterbury is seeking a Social Work Clinical Team Leader. This position focuses on clinical leadership in a collaborative health system, guiding social work practices to ensure positive health outcomes for patients. Candidates should have a Bachelor's Degree in Social Work and at least 5 years of clinical experience, alongside strong leadership skills in supervising team members. Our Community Services are mainly based at The Princess Margaret Hospital. We use a collaborative MDT approach to work alongside individuals and their support networks to manage changes in their physical and mental health.
Our Social Work team is part of a collaborative health system, aiming to achieve positive health outcomes for patients, their families/whānau, and communities. We guide people through their health journeys, whether dealing with a chronic disability or temporary issues.

The Clinical Team Leader supports and promotes the Social Work profession within our community teams, including Older Persons Therapy teams, Community Stroke, and Older Persons Mental Health. The clinical caseload of approximately 0.7FTE involves social work assessments, advice, and interventions for older adults in the North/East Christchurch region.
This role is responsible for the day-to-day professional leadership of the Social Workers in our Community Teams, providing guidance, supervision, fostering a culture of learning, leading education initiatives, and supporting service improvement projects.
