Job summary

North West Leicestershire GP Federation is seeking a Social Prescribing Link Worker to cover maternity leave across 12 GP practices. You will help patients engage with local non-clinical services and develop sustainable wellbeing plans within a multi-disciplinary team to enhance community connections.

The role requires travel to multiple locations, a full UK driving licence, and the ability to work autonomously while building partnerships with voluntary sector organisations and local authorities

Qualifications

• Excellent listening skills and empathy in patient interactions.
• Ability to manage a caseload and prioritise tasks.
• Able to travel between locations with a full UK driving licence.

Responsibilities

• Support patients to access local non-clinical services.
• Refer patients to community and VCSE organisations as needed.
• Develop and maintain networks with GP practices and partners.
