Job summary

Practice Plus Group is recruiting a Social Prescribing Link Worker to support four sites in the West Midlands region: HMP Dovegate, HMP Stafford, HMP Featherstone and HMP YOI Drake Hall. This full-time role offers an annual salary of £26,000–£29,000, with 8am–4pm or 9am–5pm hours, and weekly site visits alongside admin days at the West Midlands Regional Office.

Qualifications

• Experience in social prescribing or health/w social care settings.
• Ability to manage a caseload and provide follow-up support.

Responsibilities

• Carry out initial assessments for patients referred to social prescribing.
• Manage a caseload and ensure access to prescribed services.
• Schedule and sequence social prescribing activities.
• Liaise with Health and Prison departments to coordinate delivery.
• Ensure services align with our stated values.
