Social Prescriber Role

As a Social Prescriber, you will play a crucial role in connecting individuals with non-medical resources and support within their local community. The successful candidate aims to improve overall well-being by addressing social determinants of health and fostering community engagement.

Responsibilities include, but are not limited to:

  1. Conducting assessments to understand the social, emotional, and practical needs of individuals.
  2. Engaging in consultations to develop a holistic understanding of clients' circumstances.
  3. Connecting clients to local community resources, services, and activities tailored to their needs.
  4. Collaborating with community organizations, social services, and healthcare professionals to build a support network.
  5. Developing personalized support plans with clients, setting achievable goals.
  6. Providing ongoing guidance and encouragement to help clients meet their objectives.
  7. Building relationships with local community groups and organizations to stay informed about available resources.
  8. Organizing group activities, workshops, and events to promote social connections.

Requirements:

  • Relevant qualification in Social Prescribing or related field.
  • Experience in a similar role, preferably in healthcare or community settings.
  • Strong interpersonal and communication skills.
  • Empathy and ability to build trust with diverse individuals and communities.
  • Knowledge of local community resources and services.
  • Enhanced DBS check within the last 12 months, preferably on the update service.
