Social Care Worker
Worcester
22/04/2025 2 months initially
Monday to Friday, 9am 5.30pm, 37 hrs/week

Responsibilities

• Ensure that the outcome of all work with adults in need of care and support, and with their carers, is to promote their wellbeing.
• Carry out and review Needs Assessments & Care and Support Plans for individuals and their carers.
• Engage with people to understand the reason for their contact and their presenting needs and to consider options for resolving these at an early stage.
• Ensure opportunities for reablement and independent living are always considered and explored, enhancing individual and family resilience.
• Work with the individual in their local environment supporting them to build support within their local community.
• Ensure information and advice is provided.
• Work closely with primary care, community services, hospital teams, and voluntary community services.
• Keep and maintain records of work.
• Attend meetings and reviews where necessary.

Requirements

• A professional qualification relevant to the post such as Health and Social Care.
• Experience working with adults with complex needs.
• Experience in assessments and support planning.
• Adult DBS registered to the update service.
