We’re looking for a passionate, enthusiastic person to join our dedicated and driven team in Barrow in Furness, as we continue our ambitious programme of change within Adult Social Care. This is an opportunity to help shape the future of our service, contributing to the development and delivery of our goals to improve outcomes for the people we support.
We want you to become an integral part of the implementation phase of our newly established Long-Term Assessment and Review Service across Westmorland and Furness, supporting people aged 18 who are living in the community and have been identified as having longer term care and support needs.
This service works across health social care and third sector organisations with a primary focus upon ensuring that all support plans:
Include robust contingency plans that promote personal resilience to manage temporary changes in need or circumstance
Demonstrate how people will meet their needs and aspirations from a range of sources including friends’ family third sector the community equipment or assistive technology as well as support services
In this role you will work under the guidance and support of qualified practitioners, undertaking strengths-based care and support assessments to develop effective, person-centred support plans, while playing an active role in risk management, monitoring outcomes, and ensuring individuals receive the most effective support. Acting as a point of contact, that communities, people, parents/carers/guardians and their families can trust,
