If caring comes naturally to you and you find reward in supporting people, this could be the perfect job for you!

What we do:

Referrals are made by professionals involved in the individual's care, such as social workers, GPs, community nurses, or physiotherapists.

The team provides short-term support and reablement services to increase independence, reduce hospital or care home stays, and prevent unnecessary hospital admissions. What you'll do:

As a Social Care Worker – Rehabilitation Support, you will work actively with the multidisciplinary team, service users, and their families or carers to plan, develop, implement, and evaluate rehabilitation services, often in the service user's own home.
