You'll be on the frontline of adult social care, supporting individuals and their carers to live independently, safely, and with dignity. Using a person-centred, strengths-based approach, you'll:
Carry out and review Needs Assessments and Care & Support Plans
Promote wellbeing, re-ablement, and independence
Liaise with NHS teams, voluntary services, and community partners
Apply key legislation including the Care Act 2014 and Mental Capacity Act 2005
Identify eligibility for NHS Continuing Healthcare (CHC) and advocate for service users
Support carers and promote community-based, cost-effective care solutions
