We Do Social Work in the West Locality is seeking an experienced Adult Social Worker to support adults with care needs and carers. This post operates in a strengths-based, person-centred framework, focusing on assessment, care planning, safeguarding and risk management to achieve positive wellbeing outcomes.

The role involves working with families, carers and partner agencies to identify needs, promote independence and ensure timely, appropriate support.

Qualificações

• Qualified social worker with registration and experience working with adults.
• Experience with safeguarding, Mental Capacity Assessments, risk assessments and reablement approaches.

Responsabilidades

• Undertake strengths-based assessments for adults and carers under Care Act 2014.
• Develop, implement and review person-centred care and support plans.
• Manage a varied caseload within statutory and local timescales.
• Identify, assess and manage risks with safeguarding measures.
• Lead safeguarding enquiries, planning and reviews when required.
• Promote reablement and enablement to maximise independence.
• Advocate for adults, ensuring their views shape assessments and plans.
• Collaborate with families, carers, health professionals and partners.

About the Role

We are looking for an experiencedAdult Social Workerto join the West Locality team, supporting adults with care and support needs and their carers to achieve positive outcomes, maintain independence and improve their overall wellbeing.

This is astrengths-based, person-centred roleinvolving assessment, care planning, safeguarding and risk management. You will work closely with adults, families, carers and partner agencies to identify needs, promote independence and ensure individuals receive the right support at the right time.

The successful candidate will have experience ofsafeguarding, Mental Capacity Assessments, risk assessments and reablement/enablement approaches.

Key Responsibilities

• Undertake strengths-based assessments for adults and carers in accordance with theCare Act 2014.

• Develop, implement and review person-centred care and support plans.

• Manage a varied caseload, ensuring work is completed within statutory and local authority timescales.

• Identify, assess and manage risks, ensuring appropriate safeguarding measures are implemented.

• Lead safeguarding enquiries, planning and reviews where required.

• UndertakeMental Capacity Assessmentsand apply relevant legislation and guidance.

• Promotereablement and enablement, supporting adults to maximise their independence and achieve sustainable outcomes.

• Advocate for adults and ensure their views, wishes and desired outcomes are central to the assessment and support planning process.

• Work collaboratively with families, carers, health professionals, voluntary organisations and other partner agencies.
