Job summary

South Tyneside Council invites applications for a Social Care Practitioner in the Let’s Talk Team. This 22.5 hour per week, over 3 days, Fixed Term Contract 18 months offers a values‑driven role supporting adults, carers and families to live healthy, independent lives.

You will conduct strengths‑based conversations, assessments and support planning, working with Social Workers and health professionals to promote independence and well‑being, while maintaining accurate records.

Qualifications

• Undertake strengths‑based conversations and assess needs.
• Assist with support planning and goal setting.
• Collaborate with Social Workers, Occupational Therapists and health professionals.

Responsibilities

• Undertake strengths‑based conversations, assessments and support planning.
• Work collaboratively with people to identify goals and achieve outcomes.
• Promote independence, wellbeing and choice.
• Complete intervention work to prevent, reduce or delay ongoing support.
• Support carers and recognise their valuable contribution.
• Work with Social Workers, Occupational Therapists and partner agencies.
• Contribute to safeguarding, risk management and decision‑making.
• Utilise community resources and local networks to support people.
• Maintain accurate records and contribute to high‑quality practice.
