A local authority in Swindon is seeking a Social Care Practitioner to support residents in maintaining independence in their homes. You will assess needs, develop support plans, and engage with adults with complex needs. The ideal candidate should have NVQ Level 3 in Health and Social Care and experience in the field. The Initial Contact Team is responsible for delivering a high quality and responsive service through strength-based conversation, to understand and where possible resolve queries and issues raised at first point of contact into Adult Social Care. The Initial Contact Team offers short term involvement and by using a strengths based approach we signpost, explore all community based options and assess and arrange short term and long term support.
As a Social Care Practitioner, you will be supporting residents to remain independent in their own homes with access to effective information and advice and services that promote a strength base approach.
