Are you a dedicated and skilled Social Care Practitioner looking for an exciting opportunity to contribute to the well-being of our community? West Berkshire Council's Adult Social Care Team is seeking a motivated individual to join us as a Social Care Practitioner, reporting to the Team Leader, within our Market Street Offices.

The Role

Responsibilities

As a Social Care Practitioner, you will play a vital role in delivering high-quality social care services to adults in West Berkshire. Your key responsibilities include:

  • Working collaboratively with individuals to assess their social care needs and develop appropriate support plans.
  • Conducting reviews of existing care packages to ensure ongoing suitability and making recommendations for changes where necessary.
  • Providing information, advice, and guidance to service users and their families.
  • Collaborating with external providers to ensure the effective delivery of care services.
  • Working alongside other professionals, such as healthcare providers, to achieve positive outcomes for individuals.
  • Maintaining accurate and up-to-date records of assessments, reviews, and interventions.

The ideal candidate will possess the following skills and attributes:

  • A relevant social care qualification and experience working in a similar role.
  • Sound knowledge of social care legislation, regulations, and best practices.
  • Strong assessment and care planning skills, with the ability to identify and manage risks.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, colleagues, and external stakeholders.
  • Effective organisational and time management skills, with the ability to prioritise tasks and work independently.
  • Team player who is also able to work autonomously, demonstrating initiative and problem-solving abilities.
  • Commitment to continuous professional development and staying up-to-date with industry developments.
