Responsibilities

• To provide high-quality information, advice and assistance to individuals who contact Social Care through the Information, Advice and Assistance (IAA) service.
• To work as part of a collaborative team that responds effectively to citizens and their carers, helping them identify what matters through meaningful conversations.
• To facilitate solutions that promote independence, positive risk-taking, and social inclusion, drawing on individual strengths and community resources.
• To undertake proportionate assessments and, where appropriate, develop person-centred, strengths-based care and support plans that enable individuals to achieve their personal outcomes.
• To actively promote prevention and early intervention, supporting community resilience and contributing to community development initiatives.
