We are seeking a Social Care Practitioner to join our Physical Disability & Sensory Service Team (PDSS) within Warwickshire, for 17 hours per week.
Our team operates county wide and has bases in Warwick, North Warwickshire, Rugby and Bedworth and comprises of Social Care Practitioners and Social Workers.

Key Requirements
You will be an enthusiastic and dedicated person, preferably experienced within Adult Social Care. You will be required to work with a variety of customers, including people with physical disabilities, sensory requirements, carers and their families and other professionals.
You will be required to undertake outcome focused assessments, support planning and reviews. Duty work will also be undertaken within the role. Focusing on strengths and utilising an asset based approach, you will enable customers to identify outcomes, meet their full potential and maximise their independence.
As a strong team player, you will be required to contribute to the development and promotion of the PDSS Team, using imaginative, creative and innovative ways of meeting the support needs of our customers.
