As a Social Care Discharge Coordinator, you will collaborate with the Integrated Discharge Team to support the management of the discharge pathway for clients transitioning through the acute hospital setting. You will work closely with other agencies and professionals.

Your responsibilities include triaging and allocating referrals to suitable workers to ensure safe and timely discharges for patients on a hospital discharge pathway. You will supervise and provide complex case management advice to a team of Registered Practitioner Case Officers. Additionally, you will handle accurate data collection and escalate issues affecting team functionality to the Senior Practitioner and Team Manager.

The Integrated Discharge Team operates a discharge-to-assess model, which involves assessments outside the hospital environment. You will support clients as they navigate through and beyond this pathway.

Adult Social Care and Health services utilize modern office processes and performance management systems. The service is increasingly accessible outside traditional hours, moving towards an 8 am to 8 pm, 7-day service. Staff and managers are working on a rota basis to meet service needs.

This role requires holding a full UK Driving Licence.

The position is subject to an Enhanced Disclosure from the Disclosure and Barring Service and registration as a qualified Social Worker with Social Work England.

As a customer-facing role, the Council has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have sufficient command of spoken English/Welsh for effective job performance. Standards are outlined in the Job Description/Person Specification.
