The post holder will work alongside the Community Mental Health Services and GP surgeries under the supervision of the team manager and their leads, undertaking physical health checks, physical health clinics and general health and wellbeing assessments
The post holder will contribute to assessment of both physical and mental health needs. Focusing on the physical health and wellbeing of patients with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing
To work with other agencies to deliver a systems approach in ensuring patients physical health and wellbeing needs are met.
The post holder will undertake annual physical health checks of patients who have been identified as having severe mental illness. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests.
Assessing patients holistically. Assessing lifestyles - exercise and activities, eating and drinking, smoking, alcohol and drug use and giving advice on healthy lifestyles and signposting patients to relevant services.

The post holder will work alongside the Community Mental Health Services and GP surgeries under the supervision of the team manager and their leads, undertaking physical health checks, physical health clinics and general health and wellbeing assessments. The post holder will assess physiological observations and promote healthy lifestyles for our patients, signposting to relevant physical health interventions as required. The post holder will assist in enabling patients to access the support they need to address their physical and mental health needs and promote recovery. They will also support the locality in ensuring the Severe Mental Illness (SMI) registers are updated. The post will be part of a team working across, and on behalf of, GP practices in South East Essex.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
