A leading NHS Trust is seeking a motivated individual to work within Community Mental Health Services, focusing on the physical health of patients with serious mental illness. You will conduct health checks, promote wellness, and collaborate with various stakeholders to support patient care. Qualifications

  • At least 2 years of experience in a health-related field is required.

Job summary

The post holder will work alongside the Community Mental Health Services and GP surgeries under the supervision of the team manager and their leads, undertaking physical health checks, physical health clinics and general health and wellbeing assessments

Main duties of the job

The post holder will contribute to assessment of both physical and mental health needs. Focusing on the physical health and wellbeing of patients with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing

To work with other agencies to deliver a systems approach in ensuring patients physical health and wellbeing needs are met.

The post holder will undertake annual physical health checks of patients who have been identified as having severe mental illness. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests.

Assessing patients holistically. Assessing lifestyles - exercise and activities, eating and drinking, smoking, alcohol and drug use and giving advice on healthy lifestyles and signposting patients to relevant services.
