An exciting opportunity has arisen for a Clinical Pharmacist to develop a pioneering role within primary care. West Lancashire GP Federation is looking for a Clinical Pharmacist to join the Skelmersdale PCN.
The Clinical Pharmacist is a patient-facing role with opportunities to utilise clinical skills to improve community health. Responsibilities include managing long-term conditions, supporting hospital discharge prescribing, advising on multiple medications, and developing the pharmacist role within GP practices. The role involves acting as a specialist resource for medicines-related queries, signposting to other services, and liaising with healthcare professionals.
