An exciting opportunity has arisen for a Clinical Pharmacist to develop a pioneering role within primary care. West Lancashire GP Federation is looking for a Clinical Pharmacist to join the Skelmersdale PCN.

The Clinical Pharmacist is a patient-facing role with opportunities to utilise clinical skills to improve community health. Responsibilities include managing long-term conditions, supporting hospital discharge prescribing, advising on multiple medications, and developing the pharmacist role within GP practices. The role involves acting as a specialist resource for medicines-related queries, signposting to other services, and liaising with healthcare professionals.

Main duties of the job

We seek candidates who are enthusiastic, adaptable, with excellent clinical skills, and competent in IT. Good organisational and communication skills are essential for engaging with healthcare professionals, patients, families, and carers.
