Qualifications

  • NVQ Level 3 or evidence of previous similar study required.
  • Experience in direct work with children or adults and multi-agency collaboration.

The Children with Disabilities Service are recruiting for a permanent Short Breaks Family Practitioner.

Our Short Breaks Hub is a vital service dedicated to providing exceptional support to children and their families. We work with our partners to ensure that our children can enjoy enriching activities, develop independence, and connect with peers, while families receive much-needed respite.

As part of our mission, we aim to empower children, young people, and their families through a strengths-based approach, promoting positive outcomes, well-being, and inclusivity. Join our dynamic and supportive team to create lasting impacts!

The Post Holder’s Responsibilities Will Include:

  1. Leading and Supporting Children and Families, building strong relationships and providing advice, guidance, and timely, focused interventions that meet their needs.
  2. Conducting Assessments and Reviews, undertaking annual assessments of children and family needs, using a strengths-based and child-centred approach. Incorporate these assessments into comprehensive Short Breaks Plans and contribute to EHC Plan reviews, ensuring alignment with the best outcomes for children.
  3. Multiagency Collaboration and Decision-Making, chairing multiagency meetings, coordinating services, and presenting clear, evidence-based recommendations for decisions on care and support.
  4. Direct Work with our children, being creative in your approach adapting this in line with our children’s communication needs ensuring that their voice is central.
  5. Ensure our Parents and Carers have the right support, undertaking Carer’s Assessments and utilising community support and services.

You will need to have:

  • NVQ Level 3 or evidence of previous similar study and willingness to obtain in a reasonable timescale.
  • Experience of direct work with children or adults and multi-agency working.
  • Good knowledge of child development, as well as having an understanding of a variety of disabilities, such as ASD and ADHD.
  • Ability to work flexibly and outside core working hours, including weekends and early evenings when families are together.
  • Able to communicate in a range of different ways which are appropriate to the service user and circumstances.
  • Excellent IT skills.
  • Good team player and able to show excellent social skills.
  • Able to remain focused despite oppositional challenges from families.
