**Opportunity for 3 sessions Shared Care Salaried GP**

We are looking for a Salaried GP who will work closely with theprimary care mental health practitioners including independent prescribingnurses and clinical pharmacists with a specialist interest inNeurodevelopmental assessment and treatment.

Main duties of the job

Thejob purpose is to provide general practice input into the process of initialscreening, referral for neurodevelopmental assessment, and ongoing support inthe form of shared care with secondary care providers. They will have inputinto the physical health monitoring of people with mental illness and neurodevelopmental disorder including those with eating disorders where the patientremains under the care of specialist community teams. They will provide medicalinput, support, and supervision for other members of the primary care mentalhealth service where needed.

Job responsibilities

Duties and Responsibilities

Theday-to-day treatment decisions including prescribing will be carried out by thespecialist practitioners in the primary care team with the GP post holderadvising predominantly on comorbid physical health conditions, acting onabnormal physical health monitoring results, and participating inmultidisciplinary care for complex cases. The post holder will escalate andaccess further care where needed from specialist teams in the acute trust andcommunity services.

The GPpost holder and primary care specialists will have rapid and priority access tosecondary care specialist mental health and neurodiversity services for help,advice and escalation of care when required.

The GPpost holder will attend regular and ad hoc (in-person or remote) meetings withother clinicians either on their own or with patient(s) and carers.

The GPpost holder will have or develop a close relationship with their localsecondary care services, (as above, as well as occupational therapy,educational psychology, specialist speech and language therapy).

Theyshould be confident and competent in performing physical health assessments,have knowledge of local shared care agreements and have a good understanding ofthe medications used for ADHD and mental health problems and the physicalhealth aspects of common mental health conditions.

The postholder is not expected to be a specialist in mental health, eating disorder,ADHD or neurodevelopmental disorder but apply their generalist skills to thecare of this patient group. Risk issues will be managed appropriately byescalation to secondary care.

The GPpost holder will act as the signatory to shared care agreements between thespecialist trust and the primary care provider (STHC) in their role as theclinical presentative of this organisation

Referrals:Patients are accepted by request from primary care, step down from specialistservices and referral from other partner agencies where necessary.

Governance:The GP post holder works autonomously as a GP, employed by STHC and inpartnership with other clinical professionals employed by other NHSorganisations mental health trust and acute and community trust. The GPpost holder will liaise and coordinate care with GP practices within SouthTyneside where needed.

The GPpost holder will undertake ongoing professional development, and engagement inactivities associated with mental healthcare and ADHD management.
