Are you a dynamic and experienced leader ready to make a real impact in adult social care? We are seeking a Service Manager to lead our Transfer of Care Team and Continuing Healthcare (CHC) services. This is a pivotal role focused on ensuring safe, timely, and person-centred hospital discharges and effective CHC coordination across our system.
You will be responsible for the operational management of hospital discharge pathways, supporting five Team Managers, and working closely with the Head of Service to meet performance targets and improve outcomes for residents. You will also oversee the interface with Continuing Healthcare, ensuring robust processes and collaborative working with health partners.
This is a fast-paced, high-pressure environment that requires a confident, resilient leader with a strong grasp of adult social care legislation, discharge planning, and integrated working.
