Service Delivery Advisor

Xyla (part of Acacium Group)

Location: James Paget University Hospital (NR31 6LA) - Hybrid working

Contract: 6 Month Fixed Term Contract with potential for extension

Working hours: Monday - Friday, between 8am - 6pm, with requirements to work Bank Holidays & weekends on a rota basis

Salary: £28,000 per annum plus an amazing benefits package

We are looking for a Care Liaison Advisor to join our team, where you will support the Service Delivery Manager (SDM) and supervise a team of Care Liaison Advisors, to ensure service delivery to the specification agreed and achievement of KPIs, for Hospital Discharge and other patient flow services.

This position offers a unique opportunity to work towards quick and safe discharge plans whilst supporting families during challenging times.

Every day you will…
  • Have ownership of case allocation and be responsible for delegating to the Care Liaison Advisors
  • Complete regular quality checks to support quality assurance for clients and service users, and support care liaison advisors with training needs, including training of new services
  • Assist with recruitment, training, and development of staff
  • Build and maintain customer relationships through proactive communication and regular updates (referrers, wards, discharge teams, etc.)
  • Review and manage data analytics to help improve service delivery
