About the Role
Nottinghamshire County Council is seeking 2 Service Advisors to support the Urgent Emergency Care (UEC) service across multiple locations: Queens Medical Centre (Nottingham), Kings Mill Hospital (Mansfield), Bassetlaw Hospital (Worksop). Service Advisors will be based in Transfer of Care Hubs – multidisciplinary environments where professionals coordinate patient discharges from acute hospital settings. The role involves providing administrative and system support to ensure decisions and pathways are recorded accurately and communicated effectively. Flexibility to work across the county is essential.

Key Responsibilities

  • Accurately record discharge triage decisions and associated support plans
  • Maintain up-to-date documentation and real-time spreadsheets
  • Upload and manage documentation within the Mosaic case management system
  • Respond to phone calls and emails professionally and promptly
  • Liaise with NHS colleagues, internal teams, and external agencies
  • Support Team Managers by updating shift rotas, Microsoft Teams schedules, and SharePoint sites
  • Provide high-quality information and signposting to service users, carers, and professionals
  • Triage referrals and assist with prioritisation based on assessed needs
  • Assist in meeting service performance targets and maintaining partnership arrangements
  • Attend meetings and contribute to service improvements
  • Handle complaints in line with council policy
  • Experience & Knowledge
  • Experience working in health or social care environments, especially within hospital settings (desirable)
  • Experience managing and prioritising administrative tasks independently
  • Strong understanding of local authority social care functions
  • Familiarity with safeguarding, data protection, and adult social care frameworks
  • Experience using IT systems including case management platforms (e.g., Mosaic)
  • Awareness of national social care policy drivers (e.g., Think Local, Act Personal) is beneficial

  • A good general education, including GCSEs (A–C) in English and Maths or equivalent qualifications
