Job summary

East of England Community Health and Care NHS Trust is recruiting a Senior Urgent Care Triage Practitioner. The role involves triaging referrals, coordinating care pathways for patients in crisis, and supporting early discharge to reduce hospital admissions, within an integrated, system-wide team.

Qualifications

• Current NMC or HCPC registration.
• Adult Nursing BSc or Paramedic Sciences BSc.
• Evidence of continuing professional development.
• Experience in Advanced Consultation skills.
• Assessor/mentorship preparation; ENB 998 or equivalent.

Responsibilities

• Triage patients referred to UCCH as part of the multidisciplinary team.
• Assess and prescribe care pathways for patients in crisis with long-term conditions and frailty.
• Support early discharge and prevent unnecessary admissions.
• Work with health care professionals and agencies to provide integrated service.
• Assist activity within UCR Hub (triage) and Community Virtual Ward.
• Arrange care interventions and assessments for patients referred to UCCH/UCR/VW.
