Job description

An exciting opportunity has arisen for a permanent band 7 Senior Urgent Care Practitioner to join the community team to supporting Ceredigion Same Day Urgent Care/Outreach service.

This post will require an enthusiastic, motivated and dynamic practitioner that has a broad understanding and experience of acute and chronic conditions. Experience of autonomous practice in the emergency/urgent/MIU arena is essential.

The team has the single goal of ensuring the best outcomes for our patients. We actively encourage continuing professional development, leading to the enhancement of your clinical skills and theoretical knowledge. You will have the opportunity to develop and consolidate skills in advanced clinical assessment and treatment and be involved in improving integration across the community with our proposed same day emergency care provision.

Main duties of the job

The service is constantly developing, in line with changes in local and national healthcare provision and as such we are looking for a forward- thinking, competent, and caring Practitioner with the skills and experience needed for triaging and assessing patients and carrying out treatments working closely with the ACP's and the community MDT.

The post holder will be expected to undertake in depth advanced assessments and have the experience and training to develop care plans with the community/primary and acute MDT. Diagnostic skills and the ability to multi-task are essential.

The successful candidate will be committed to service improvements with an acceptance of change, and desire to work in a positive environment that embraces the challenges urgent care faces as the service moves forward. You will demonstrate a calm and confident approach to your work, demonstrating excellent clinical and communication skills and the ability to manage and prioritise workloads effectively. Team working and the ability to be flexible is important to ensure service provision can be delivered efficiently and effectively.

The role is currently Mon-Fri 7.30am-8pm but extended hours and a 7-day service is planned for the future.

Qualifications and Knowledge
  • Registered Practitioner (e.g NMC/HPC)
  • CPD to Masters level
  • Relevant Level 7 Module covering Advanced assessment, Clinical Examination and diagnosis skills for minor injury and illness presentations
  • ALS / EPALS and in-house Trauma Training or ATLS
  • Evidence of continuing professional development
  • Awareness of Clinical Governance
  • A&E Nursing / Urgent Care / Community or Primary Care experience
  • Able to relate theory to practice
  • Maintain their personal and organisational development
  • Provide specialist clinical advice within the multidisciplinary team
  • Administers agreed drugs in accordance within locally agreed patient group directions (PGD's)

Experience
  • Relevant experience as a Registered Nurse in the field of nursing minor injuries/illness/Urgent/Community or Acute Care
