Job summary
In North Tyneside, the CLDT is part of Northumbria Healthcare NHS Foundation Trust. It is a multi-disciplinary team managed along with services provided by the Local Authority.
Building on the current CLDT provision, this post will facilitate the provision of children and adult specialist service offering assessment, intervention, and training for people who have a learning disability, complex behavioural/and or mental health needs.
This post involves a range of clinical activities, including supporting senior nurse with monitoring of forensic and behavioural case load, assessments and interventions and managing risk in the community. Liaising and working in collaboration within the multi-disciplinary workforce.
The ideal candidate will be passionate about working in collaboration with service users, their family, carers, and professional colleagues to enhance health and well-being.

This post will be based with the Community Learning Disability Team in North Tyneside. The post holder will work within the Enhanced Forensic Positive Behaviour Support Team. The post holder will provide and support in highly specialised risk assessments and interventions for adults who present with a forensic history/presentation.
The post holder will assist senior staff with the development of highly specialised Positive Behavioural Support Plans for patients and service providers to reduce or prevent placement breakdowns or admissions into inpatient settings.
The post holder will provide support with the development of highly specialised nursing care and Positive Behavioural Support plans for adults living within the North Tyneside area who present with complex behavioural/mental health and forensic needs. The post holder will carry out behavioural and risk assessments, observations and interventions under the direction of a senior nurse.
There is an expectation the post holder will contribute to all relevant meetings and be involved with service development, audits and quality improvements.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
