About The Role

We operate a 34-bed home supporting individuals with learning disabilities, dementia, and mental health diagnoses, providing transition support from hospital back into the community. Our team helps create a lively, supportive environment with activities like gardening, office work, parties, bingo, music, and pizza-making.

As a Support Worker, you will provide high-quality, enabling care, facing new challenges daily and making a real difference.

Key Responsibilities:

1. Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
2. Develop engaging activity plans that promote autonomy and personal growth.
3. Support individuals in maintaining healthy lifestyles and managing health needs.
4. Facilitate effective communication and maintain detailed, person-centered records.
5. Oversee medication processes and support self-administration.
6. Maintain a safe, clean environment and adhere to health and safety protocols.
