Post: Senior Support Worker

Hours: 37 per week (permanent)

Salary: Grade 6

Location: Brierley Hill Health and Social Care Centre or various locations across Dudley borough

An excellent opportunity has arisen to work within Assessment and Independence as a full-time Senior Support Worker. Your primary role will be to support the hospital discharge process. You will be working a set rota pattern which includes weekends, evenings, and bank holidays, for which relevant enhancements will be paid.

The Role

Your main duties will include:

  1. Assisting in managing the demand for care capacity and ensuring safe discharge and care provision.
  2. Working on computer systems to support the discharge process.
  3. Liaising with hospital professionals to ensure safe discharges.
  4. Liaising with care agencies, Service Users, and family members to ensure safe discharges.
  5. Accurate recording on case notes.
  6. Accurate recording on spreadsheets to ensure detailed reports can be completed.
  7. Re-start of care for Service Users of Dudley.
  8. Being involved in a duty rota and having knowledge of this.
  9. This post is subject to an enhanced DBS check.
