Swanton Care and Community is seeking a Senior Project Worker (Supported Living) in Middlesbrough. The role involves leading day-to-day shifts, providing practical and emotional support, and empowering individuals to live their chosen lives.

A valid UK driving licence is desirable, and you will work across varied shifts, including days, evenings and weekends. The ideal candidate will have care experience, be empathetic, and capable of safeguarding and promoting independence.

Qualifications

• Ability to demonstrate equivalent of Level 2 in English.
• Willingness to work towards Care Certificate.
• A good standard of general education.
• 2 years of working within a team.
• Experience in a care environment.
• Experience, either paid or voluntary, supporting people with learning disabilities, autism or mental health needs.

Responsibilities

• Support individuals with their own choices and help them to lead a meaningful life.
• Act as a Key worker and ensure team works in the best interests of the individual.
• Ensure health and wellbeing, advocate on behalf of the individual.
• Record outcomes in service user records and maintain confidentiality.
• Maintain medication management including ordering, storing and administering.
• Undertake audits related to supported living and participate in team meetings.
• Encourage engagement in activities and promote independence.
• Communicate in the individual’s preferred method and be mindful of cultural needs.
• Participate in supervision, appraisal, and mandatory training.
• Safeguard health, safety, and welfare and escalate concerns as needed.
• Undertake lone working duties and have driving licence (desirable).
