Job description

We are looking for an enthusiastic and committed person tojoin the Nottingham Recovery Networks Recovery Case Management Teams.

The post holder will be an integral member of the NottinghamRecovery Network, ensuring effective joint working across all areas enablingservice users to move through the system and progress out of treatment andreintegrate as quickly as possible.

The post holder will provide high qualityevidenced based substance use treatment to adults with dependent drug use inline with national guidance. This willbe achieved through assessment, screening, care planning, recovery planning(SMART), case management, the delivery of comprehensive, structured andunstructured treatment and implementing and evaluating treatment plans in linewith Clinical Guidelines

Main duties of the job

To provide, and supervise the delivery of high standards ofnursing care for the treatment and support of substance dependency and complexhealth needs resulting from substance misuse in accordance with up-to-dateevidence based professional practice, national guidelines and Frameworkpolicies.

The main focus of this post is to work as amember of a multidisciplinary team who cover the unit 365 days a year and whoare responsible for the delivery and monitoringof high standards of all relevant substance misuse andhealthcare interventions to residents.

Working within a health improvement framework,the post holder will provide assessment, brief interventions, prescribedtreatment, case management, crisis intervention, and all aspects of requirednursing care. In addition, the post holder may facilitate access to primarycare and mental health services, monitoring and implementation of agreedtreatment plans which aim to improve the general physical and psychologicalhealth and wellbeing as required in the context of behaviour change leading tohealthier lifestyle. The post-holder will be expected to manage their own timeeffectively and offer co-ordinated support to junior staff and students,allocating work appropriately in order to best achieve desired positiveoutcomes.

About the Service

The Nottingham Recovery Networkaims to delivera fully-integrated, high quality drug and alcohol treatment and recovery supportsystem that is locally owned and personally valued, with service user and carerinvolvement embedded in all aspects of its practice. The Nottingham RecoveryNetwork comprises of Framework Housing Association (Lead Agency), Double Impactand Nottinghamshire Healthcare NHS Foundation Trust.

The service focuses on a three phase servicedelivery model comprising of:

-Recovery Orientation (Engagement)

-Recovery Delivery Interventions (Including psychosocial and pharmacological treatment interventions)

-Re-integration (Recovery including the Double ImpactAcademy)

This is underpinned by the four cornerstones of:

-Peer-led MutualAid

-Mentoring and Volunteering

-Service User Involvement

External Mutual Aid

Purpose of Job

Thepost holder will be an integral member of the Nottingham Recovery Network,ensuring effective joint working across all areas enabling service users tomove through the system and progress out of treatment and reintegrate asquickly as possible.

Thepost holder will provide high quality evidenced based substance misusetreatment to adults with dependent drug misuse in line with nationalguidance. This will be achieved throughassessment, screening, care planning, recovery planning (SMART), casemanagement, the delivery of comprehensive, structured and unstructuredtreatment and implementing and evaluating treatment plans as defined byModels of Care 2002/2006, NICE guidelines quality standards for drug usedisorders 2012 (51 & 115) and public health guidance 24.

Thepost holder will have responsibility for providing support and guidance tosubstance misuse workers.

Thepost holder will be responsible for nursing related tasks which may includeassessing patients, administering medication including IMinjections, care planning and treatment, monitoring patient progress,communicating with patients and their families, and coordinating care withother healthcare professionals.

Thepost holder will proactively maintain contact with service users striving forpositive outcomes for all to include where possible theattainment or maintenance of abstinence.

Scope of Job

To work in partnership with the wider Nottingham Recovery Network teamto case manage more complex cases and support individuals with substance misuseproblems through the recovery system.

To carry a caseload of service users with dependent drug misuseincluding multiple needs.

Nursing related tasks which may includeassessing patients, administering medication includingIM injections, care planning and treatment, monitoring patient progress,communicating with patients and their families, and coordinating care withother healthcare professionals

The post is required to be competent and autonomous in implementing andevaluating treatment plans for more complex cases and will have the experienceand knowledge to competently advise more junior members of the team.

Qualifications

Post holder must hold a current nursingqualification and be registered with the NMC.

Preceptorship offered to newly qualified nurses.

Key Responsibilities

Manage acaseload of clients ensuring a high standard of clinical care for thepatients/clients under your management. This will include management of serviceusers prescribed OST.

Work as amember of the team, be flexible to the needs of the team

Toadminister medication including IM injections and monitor patients clinicallyas required i.e. BP monitoring.

To managethe clinical area safely and respond appropriately in line with NRN procedures.

To be partof the on-call rota providing a response to service users and professionals inthe absence of the key worker

To providesupport and guidance to more junior members of the team.

To carry acaseload with responsibility for assessing, planning, implementing andevaluating care programmes to identify levels of dependence and create anindividually tailored plan of care; to encourage the development of workingtowards the achievement of agreed goals.

Tocomplete a full risk assessment of need including risk to self and others andadult/child safeguarding identification and risk management.

To linkservice users to appropriate internal and external services and other areas ofthe Nottingham Recovery Network.

To providesupport to shared care and satellite clinics.

Toproactively maintain contact with service users instilling belief ofRecovery.

To ensurethat appropriate Mentor or Peer Mentor support is in place to support theservice user on their journey to recovery.

To assistservice users to access learning, training and development opportunities whereappropriate.

To utiliseskills and knowledge to facilitate therapeutic one to one and grouppsychosocial based interventions and learning opportunities alongside othermembers of the team.

To utilisemotivational interviewing and CBT techniques in one to one settings to promoteengagement in treatment services, encourage self-esteem, well-being,self-responsibility and enhanced motivation.

To promotehealthier lifestyles and increase service user choice and engender a recoveryfocused community

To promotemutual aid and self-help.

To take anactive role in multidisciplinary meetings, to feedback, record and discussindividual cases.

To trainas a practice supervisor/assessor and provide mentorship and training tostudent nurses, social workers, other professionals and new staff to developand improve awareness and knowledge of substance related issues.

To workclosely with all partnership members for the benefit of the service user.

To dealwith any safeguarding concerns in line with policy and procedure.

To ensurethat all aspects of confidentiality are adhered to

Administration & Systems

As a treatment system, patient notes areconsidered health care records and are therefore subject to enhancedscrutiny. Record keeping must not onlyadhere to Nottingham Recovery Networks record keeping policy, but alsonational guidance on health care records (NMC)

To ensure that all administration dutiesconnected with Nottingham Recovery Network and Double Impact programs arecarried out.

To ensure that key performance indicatorsare met as stipulated by the service level agreement and to achieve specifiedtargets as set by the team leader.

To operate record keeping and monitoringprocedures in accordance with contract requirements. This will involve the recording of individualactivity in accordance with agreed data collection systems

Person Specification
Qualifications
  • At least a level 3 Diploma in Health & Social Care
  • Degree level knowledge in Health/Social care or other relevant applicable field
  • Training/Qualifications in another relevant area e.g. counselling, psychotherapy etc
Abilities
  • Ability to provide and receive complex sensitive or contentious information where persuasive, motivational, negotiating, training, empathy or reassurance skills are required.
