We are seeking a dedicated, motivated, and skilled Band 6 Senior Staff Nurse to join our dynamic Rapid Response Team. This role involves developing pathways for Same Day Emergency Care in the community, working alongside Adult Community Services, Social Care, Acute Trusts, and Primary Care colleagues.
The team, part of the SE London Accelerator Programme, delivers Urgent Community Response pathways and has been operational since 2013. It comprises Nurses, Occupational Therapists, Physiotherapists, Social Workers, and support staff, providing holistic assessments for adults over 18 registered with a Bexley GP and residing in Bexley. The team operates from Queen Elizabeth Hospital, Woolwich, 7 days a week, 8am-8pm, focusing on rapid assessments with short-term interventions.
The post involves working within a multidisciplinary team, conducting comprehensive nursing assessments, managing clinical risks, and providing specialist advice. The role includes managing breast drains, wound care, and IV antibiotics, working across hospital and community settings, and supervising junior staff.
