Senior Staff Nurse

A development opportunity has arisen for Community Staff Nurse to join the newly formed Severe Multiple Disadvantage Team that works across the County providing both physical and mental health support to a very vulnerable cohort of patients who face multiple barriers to access healthcare.

You must have significant community experience with the ability to motivate yourself and others. It is essential you have experience of working in both physical and mental health settings and be committed to supporting integrated ways of working across our community as the team works very closely with other agencies for example the Street Outreach Team. The service also works in close partnership with primary care.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Main duties of the job

You must be able to maintain the highest standards of quality care for your patients. You will be expected to manage a caseload of complex, high risk and support clients to manage their health needs . You will therefore demonstrate excellent communication and interpersonal skills with the ability to manage and lead change within the community. You will be required to work in different environments often engaging with clients who are living in temporary accommodation or on the street,

You will need to work autonomously within a multidisciplinary environment and be politically aware of service developments. You will be flexible, reliable and approachable.

Job responsibilities

Applicants must be 1st Level Registered General Nurse or Registered Mental Health Nurse with current NMC Registration.

You must have experience of working across both physical and mental health. The role requires a clinician with specialist practice skills in both physical and mental health to assess, plan, implement and evaluate patients and carers complex needs. They need to be able to critically analyse and develop treatment plans within a multidisciplinary framework and within current policy, best guidance/practice and research based evidence.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

  • Holds a mentorship qualification and has ability to provide mentorship to others
Contractual Requirements
  • Ability to work to meet the needs of the service (including unsocial hours and bank holidays) as identified within the contract of employment
