CSH Surrey NHS is seeking a Band 6 Speech and Language Therapist to join the community team in Woking. The post involves assessment, diagnosis and therapy for adults with communication and/or swallowing difficulties arising from neurological conditions and frailty.

You will be part of an integrated MDT, receive regular supervision, and have access to CPD opportunities. A car driver is essential to cover a wide geographic patch and provide services across settings.

Job summary

We have an exciting opportunity for a Band 6 Speech & Language Therapist to join our friendly community team. We provide specialist assessment, diagnosis and therapeutic interventions for adults in the community who have communication and/or swallowing difficulties, resulting from acquired or progressive neurological conditions such as stroke, motor neurone disease, Parkinsons and dementia as well as frailty. We work as part of an integrated multi-disciplinary team in the North-West Surrey Neighbourhood Boroughs.

Main duties of the job

We are a team committed to delivering high quality patient care, including a community FEES service. Our clinicians have wide ranging expertise, and peer support is always readily accessible. The post holder will have access to regular clinical supervision and CPD opportunities in line with their agreed personal development plan.

This post would suit a Band 6 therapist or an experienced Band 5 therapist who is keen to work towards developing to Band 6. Completed dysphagia training is preferred but therapists who can evidence significant progress towards completing their dysphagia competencies will also be considered. The successful candidate will join a team of highly skilled Speech & Language Therapists, delivering services in community inpatient, outpatient, and domiciliary settings. As this is a community service that covers a large geographical area, being a car driver is essential.

Job responsibilities

Purpose:

• To provide specialist assessment, diagnosis, and therapeutic interventions for patients on the rehabilitation ward at Woking Hospital, in outpatient clinics, and patients in the community who have communication and/or swallowing difficulties, resulting from a variety of acquired or progressive neurological conditions or frailty.
• To provide advice, support and education for patients and their families/carers, and offer training to students, and other Health Care Professionals.
• To be able to work independently and as part of a Team, and to participate in innovative projects and service development to benefit patients and colleagues.

Patient Care Responsibilities:

• Be able to work autonomously, with a specialist, often complex caseload, to assess, diagnose, develop and implement programmes of care. Supported by clinical supervision and SLT Lead.
• Assess, differentially diagnose, formulate treatment plans (in collaboration with patients and carers), write assessment reports, identify and choose appropriate therapeutic or clinical management techniques from a range of options, provide appropriate therapeutic intervention, and evaluate treatment outcomes.
• Demonstrate clinical effectiveness by use of evidence-based practice and outcome measures.
• Provide complex and sensitive information to patients in a manner that they can understand e.g. regarding cognitive changes
• Refer on for specialist assessment for Assistive and Augmentative communication. Liaise with specialist centres to provide continuity of care and help source funding for equipment.
• Liaise with a wide range of professional colleagues and other agencies to ensure comprehensive management of the patient e.g. attendance at ward meetings and case conferences and telephone liaison e.g. with GPs, dieticians and social services.
• Plan for patient discharge, ensuring appropriate onward referral and liaison as required.
• Adapt practice to meet individual patient circumstances, including due regard for cultural and linguistic differences.
• Complete incident forms where appropriate and discuss pertinent issues regarding safeguarding/incidents with Speech & Language Therapy Lead and others involved.
• Work with patients with a variety of diagnoses and their carers/families, across the teams geographical patch. Working flexibly to provide an equitable service to all patients, as the caseload determines and to cover periods of staff absence
• Work with a designated caseload and be flexible to carry out work in other areas of the wider SLT Team to facilitate equity of service provision for all patients (according to level of experience and competence) as requested by Speech and Language Therapy Lead, and to further develop own professional practice.

• Knowledge and experience of working with adults with dysphagia &/or communication difficulties
• Minimum of 18 months working at a junior level with patients in specialist area e.g. Adults, Neurological patients with communication and/or swallowing difficulties
• Experience of assisting with training and supporting students
• Excellent experience of setting patient-centred goals & providing outcomes

Desirable

• Experience of working on wards or in outpatient clinics
• Ability to transcribe speech phonetically
• Evidence of contribution to publications and/or locally known within field of emerging expertise
