Senior Specialist Nurse - Breathe Service

JOB SUMMARY The post-holder will lead the delivery of the Breathe service in community To oversee the management of defined clinical caseloads, providing specialist support, education and complex case management to people with respiratory conditions and their families and carers Work as part of a wider Neighbourhood Multi-disciplinary team of nursing, therapy, mental health, social care and primary care staff. Liaise with primary and secondary care colleagues, providing education and support, thus developing and maintaining high quality evidence based respiratory care across Barnsley KEY RESULT AREAS: Work autonomously assessing and managing own clinical caseload for all clients/patients within a designated neighbourhood (both clinic and home visits). Supporting patients with a range of respiratory conditions or on a respiratory rehabilitation pathway (including, but not limited to coronavirus rehabilitation, patients requiring End of Life support or those requiring Long Term Oxygen or nebuliser assessment and therapy) Use and apply specialised skills underpinned by advanced theoretical knowledge of professional practice in order to maintain professional competence and fitness to practice as a Respiratory Nurse Specialist To work as the Lead expert practitioner within the Breathe Service demonstrating evidence based knowledge, investigative and analytical skills and specialised assessment techniques to triage, assess and manage a range of complex presentations , taking into account physical, psychological and social care needs. Formulate and implement treatment plans based on current evidence and expert clinical reasoning demonstrating a comprehensive understanding of treatments including the integration of the principles of self-management, enabling individuals to function independently by increasing their choice to remain in their own home/community, thereby reducing inappropriate hospital admissions Enable people to receive an early and accurate diagnosis and receive the relevant information and support to manage their respiratory disease. Lead multi-disciplinary team meetings, contributing to decisions with regards to patient care Work alongside Primary Care GPs and Practice Nurses and in managing patients within the respective Primary Care Network. Providing targeted support to practices, where rates of respiratory disease per practice population are highest. Work in collaboration with the respiratory and medical teams within secondary care to help facilitate the early supported discharge of patients following an emergency admission for an acute or chronic respiratory condition, to support the provision of seamless care. Manage referrals to the service from medical and nursing staff and other sources and ensure patients are seen within the required timescales based upon clinical need. This will include patients in crisis requiring urgent assessment and support, those at risk of hospital admission and End of Life patients. Play a Lead role in initiating, planning and implementing induction, training and education of junior team members, the broader community nursing teams and students Ensure appropriate supervision and caseload support of junior members of the team Understand the requirements and implications for service delivery of National Standards eg NICE guidelines, National Service Frameworks, British Thoracic Society guidelines where applicable Lead on the development and delivery of specialised education programmes to own and other professionals to enable best practice in the care and treatment of individuals with respiratory conditions Adhere to and as appropriate contribute to the development and review of policies, procedures and standards relating to the service; using an evidence based approach. Work within and as appropriate contribute to the clinical governance framework to maintain consistently high standards of care, working from an evidence base or current best practice. Undertake Nurse Prescribing in response to assessed need and in accordance with trust policy. Ensure complete, accurate and up to date records are maintained in accordance with NMC guidelines and trust policy. Record all client contacts and activities, and input data into appropriate IT system in a timely manner in accordance with PCT protocols. Adhere to the NMC scope of professional practice and trust policies and procedures; ensuring professional competency and development by updating skills and knowledge; attending all mandatory training. Communication with others: Communicate effectively, empathically and in a cultural sensitive manner with patients, representatives, carers and their families, interpreters will be used effectively where the need is identified. Communicate effectively with all members of the Breathe Service; providing information as necessary to provide a cohesive and effective service. Communicate effectively with other health professionals, statutory and voluntary agencies to ensure a team approach to patient care; providing explanations of any relevant cultural issues as required.
