What you will do:

  • Work within relevant legal frameworks, guidance, policies, and procedures to undertake assessments, reviews, and support planning with individuals and their carers, promoting well-being, independence, and safety.
  • Enable individuals to identify their own needs and the outcomes they wish to achieve, implement, and review support plans to meet these needs.
  • Liaise with other professionals and agencies as required to ensure a comprehensive approach to the service.
  • Provide guidance and support, offering supervision and line management to registered and non-registered social care professionals, ensuring quality and performance standards are consistently met.
  • Engage directly with customers who have a learning disability, physical disability, long-term conditions, are young carers, or are transitioning into adulthood and support older people to live safely in the community.
